VC: 5 Elements Index Card

I adore index cards, which I think used to be called SKINNIES in the crafting world! Let's make one for this swap that includes 5 required elements (plus anything else you'd like to add). The 5 I've chosen are: lace or doily (paper doilies are absolutely fine); a the image of a vintage photograph; a sentiment; a number; and finally, a metal embellishment (a key, a charm, a filigree corner embellishment, etc.). For this swap we will be using 3x5" index cards.
